Site Observations
Site observations are records of hazards or other safety-related problems on your site. They will help you to:
- Get a picture of the kind of safety issues that arise on your site
- Track problems to ensure they are fixed before an accident happens
- Document the corrective actions that have been taken for each identified problem
The Site Observations page can be accessed from the home screen and contains two buttons for making and viewing site observations.
Making a new site observation
1. Enter general details
- Site - Select from list. By default the current site is selected.
- Date of observation
- User making observation - Select from list. By default the signed in user is selected.
- Description of what it is you’re observing
2. Add a photo
Tap the camera icon or grey text to add a photo of the observation BEFORE any actions are taken. The photo can be taken using the built-in camera or by selecting a photo from your local gallery.
3. Make safe and / or notify
Next, select what you’re going to do about the hazard you’ve observed:
- Make Safe: Choose this if you are able to resolve the issue and make the hazard safe yourself
- Make Safe and Notify: Choose this if you are taking actions to make the hazard safe, but also want to alert your supervisor that further action may be required
- Notify: Choose this if you are unable to deal with the problem yourself and want to notify your supervisor directly
If you choose a ‘make safe’ option, you will record the actions that have been, or will be, taken to resolve the issue:
4. Add actions
- Tap the “Add Action” button to enter action details.
- Describe action that will be taken by entering text into the first box.
- Select assignee to carry out action. By default the signed in user is selected.
- If the action has already been carried out, select the date the action was completed
- More actions can be added using the “Add Action” button and the current action can be removed with the “Delete” button.
5. Add mitigation photo
Tap the camera icon or grey text to add a photo to show evidence that the hazard has been mitigated. The photo can be taken using the built-in camera or by selecting a photo from your local gallery.
6. Close out observation or notify a supervisor
If you believe the actions you have taken are sufficient to make the hazard safe, tap “Close Out Observation” and select “continue” on the prompt to close out the observation.
The app separately records close out by the original observer and by a site supervisor:
- If you are the original observer and you select ‘close out’, you won’t be able to edit the observation any more, but it can still be changed by a supervisor
- If you are a supervisor and you select ‘close out’, the observation will be closed for editing and can only be opened in ‘view’ mode from then on
If you are unable to fully resolve the issue, select “Notify a Supervisor” and tap “continue” on the prompt to indicate that further action is required. A list of supervisors will be shown, allowing you to select the relevant supervisor to notify.
Viewing existing observations
From the starting page, tap the “View Existing Observation” button to view and manage site observations. The listed observations show the date the observation was recorded, the description of the observation and an indication of if the hazard has been closed out. If an observation is closed out by the observer and/or a supervisor, a check mark will appear below the O and/or S respectively. An arrow (as in the example below) indicates that the original observer has notified a supervisor.
From the list, any observation can be selected to edit, add actions or be closed out using the same process as outlined in the previous section.
